Selecting below confirms I approve of my child’s participation in Vibe Performing Arts programs. I will not hold Vibe Performing Arts liable for any injuries occuring as part of the program, and certify that he/she has health insurance and is in good health and able to participate in the programs. If medical attention is required for illness or injury, I grant permission for such care to be rendered. I also accept the terms of the COVID-19 waiver from liability.
